You may request services by calling the Visiting Nurse Association at 724-463-6340.

Referrals for services are accepted from physicians, community agencies, family members and friends. Following the call a nurse will visit you at your home, make an assessment, establish a plan of care, and contact your doctor for approval to make regular visits.

If you are in a hospital or nursing facility, plans can be made prior to discharge. This can be done by the doctor, nurse or social worker.
