Visiting Nurse Association Extended Home Care services are primarily provided to patients who do not meet strict medical necessity criteria.

Referrals are accepted from doctors, family members, friends and community agencies. Once the referral is made, a nurse will visit the patient's home, establish a plan of care, and contact the patient's doctor.

Payment is self pay, or third party long-term care insurance. Through contractual arrangement with Aging Services, Inc., VNA Extended Home Care services are also offered to those who meet non-Medicare eligibility requirements.
